How to apply

Applicants for PHCDC grants must complete the appropriate application form to apply. Please download the required fillable PDF form below, complete it by filling in the information on your computer or by hand, save the file and email, mail or deliver it to the PHCDC office. 

Before filling out the form, please carefully ready the guideline documents below. When an approved project is completed, follow-up reports must be completed.

Those requesting a donation must fill in the Donation Request Form.

Grant application deadlines

• April 15 at 4:30 pm
• July 15 at 4:30 pm
• October 15 at 4:30 pm
• January 15 at 4:30 pm

If the deadline date lands on a weekend, it will be the following business day at 4:30 pm. Please note that the board will be meeting approximately three weeks after the allocation deadline.
