How To Apply

Applicants for PHCDC grants must complete the appropriate application form. Please download the required fillable PDF form below, and complete it by filling in the information on your computer or by hand. To submit, you may email, mail or deliver it to the PHCDC office. 

The Application Guidelines Form will provide all necessary information for a successful submission. When an approved project is completed, follow-up reports must be completed.

Those requesting a donation must fill in the Emergency Donation Request Form.

Grant application deadlines

• April 15 at 4:30 pm
• July 15 at 4:30 pm
• October 15 at 4:30 pm
• January 15 at 4:30 pm

If the deadline date lands on a weekend, it will be the following business day at 4:30 pm. Please note that the board will be meeting approximately three weeks after the allocation deadline.

When signing forms electronically, please use the E-sign tool or Fill & Sign tool available in your PDF viewer's tool menu. Add your signature by drawing/writing in the signature box. Click "Apply", and place your signature on the appropriate line of the form.
